Insurance and Billing

This information is provided to help you with the insurance and billing questions you might have related to your visit to the Spine Center at Saint Francis Memorial Hospital.

If you are uninsured or do not provide evidence of insurance, please be prepared to pay at the time of service. If insured, you will be required to pay any deductible or co-payment at the time of service.

Insurance Plans Accepted:

Saint Francis Memorial Hospital accepts the majority of insurance plans available. Plans accepted vary for each physician, however special arrangements may be made if your plan is not accepted by the physician of your choice. For more information please contact us and we will be happy to answer your specific questions.

For your convenience, we will bill your insurance company and collect payment. Any portion of the fee not covered by your insurance company will be billed to you. You will receive monthly bills including the amount billed to your insurance company, until the balance is paid.

Arrangements regarding your insurance plan and benefits to which your are entitled are between you and your insurance carrier. Full payment of your bill is your responsibility.

Medicare

Our office is a participating provider with Medicare. We will bill you for your deductible and co-payments after Medicare pays. Please bring your card for Medicare and any supplemental policies.

Workers' Compensation

If your problem is covered under a Workers' Compensation claim, it is necessary that your have full claim information and prior authorization from the insurance carrier before your office visit. We will need your:

• claim number
• carrier name and address
• date of accident
• employer information

Personal Injury Claims

If your problem relates to an auto accident or other insured injury, please let us know. Payment is due at the time of service, and we will provide the necessary paperwork for you or your attorney. We do not accept liens.

Disability Forms

We will fill out your disability forms. In order to research the required information and complete the form, we require a $10 processing fee and five business days.

Pre-Certification:

Certain services may require pre-approval from your health insurance company before they can be performed. Learn more about Pre-Certification requirements.

First Time and Surgical Patient Registration:
